When you submit our consultation request form we receive your name, phone, email, ZIP code, main concern, preferred contact time, and any optional note, plus the page you submitted from. Please do not send medical records, diagnoses, or other sensitive health details through the form. We share your request with a local hormone health provider in our network so they can contact you about a consultation. Like most websites, our host may log basic technical data such as IP address and browser type.
How we use it
We use form information to respond to your request and may share it with clinicians or referral partners in your area for that purpose only. We do not sell your personal information.
Service providers
Form submissions are processed by FormSubmit (formsubmit.co) and delivered by email. Their handling is governed by their own policies.
Your choices
You can ask us to access or delete your information, or to stop contacting you, by emailing leads@balancehormones.org. California residents may have additional rights under the CCPA/CPRA.
Children
This site is intended for adults and is not directed to children under 13.

We may update this policy and will change the effective date when we do.
